For Catholics, the Sacrament of Baptism is the first step in a lifelong journey of commitment and discipleship. Whether we are baptized as infants or adults, Baptism is the Church’s way of celebrating and enacting the embrace of God.
Please download the Baptism request form, complete the form, and email it back to us at holyangels7558@gmail.com or mail it to the office at: Holy Angels Parish, PO Box 166, La Valle, WI 53941 to begin your baptism preparation.
You will need a Godparent verification form for each Godparent. You should download that form and have each Godparent fill out the top portion of the form and then they should take it to their local priest and have him sign and stamp it. That church will mail it back to us, as directed on the form. Once the Baptism request form and the Godparent verification forms from the Godparents are received, Father and you will decide on a date and worship site for the Baptism.
Parents will need to meet with Father for Baptism preparation if you have never gone through Baptism prep before. You will need to contact the parish office at 608-985-7558 or email holyangels7558@gmail.com to set up this meeting.
